I am writing to share some important news regarding our practice and your ongoing healthcare. After much consideration, I have made the decision to transition out of our practice at College Station Surgical Associates. Serving you and being a part of your healthcare journey has been an incredible privilege. I am deeply grateful for the trust you have placed in me over the years, and I will truly miss the wonderful relationships I’ve built with my patients here.
Your Continued Care is in Excellent Hands
My primary concern during this transition is ensuring that you continue to receive the highest quality of care. That is why I am so pleased to recommend my partner, Dr. Williams for your general surgery needs. I have the utmost trust and confidence in Dr. Williams’ skills, compassion, and dedication to medicine. Because we have practiced together, he is already familiar with our clinical standards, and I know he will treat you with the same care and respect you have grown accustomed to.
- To stay with this practice: You don’t need to do a thing. Dr. Williams is fully prepared to take over your care seamlessly, and your medical history is already securely available to him.
- To follow me or choose a new local provider: I completely understand and want to support whatever choice is best for you and your family. If you wish to continue seeing me, please call my assistant, Marla at 254-404-2555 to set up an appointment.
- If you are a weight loss patient (surgical or medical), or a reflux patient, we are developing a process to support your long term follow up needs through a telehealth process. Please call Marla at 254-404-2555.
How to Request Your Medical Records
If you choose to transition your care to a new provider outside of this office, you will need to authorize the release of your medical history. Here is how to do that:
- Contact the office at 979-977-3490.
- Request and complete a Medical Records Release Form.
- Once returned, our administrative staff will promptly transfer your records to your new provider.
Note: If you choose to remain with Dr. Williams, you do not need to request your records. They will stay right here in the office system.
