Preparing for a Well Visit

In preparation for your child’s Well Visit, please be aware of the following:

  1. Due to COVID-19 precautions, a max of 2 persons in addition to the patient are allowed to attend the visit.
  2. Please make sure that everyone 2 and older is wearing a non-vented mask, covering both nose and mouth.
  3. For well visit appointments ages 1 month to 3 years, developmental screenings/CHADIS questionnaires must be completed prior to the appointment.
  4. COVID screening at the door is required. If you or your child are ill and you have an appointment for a wellness visit, please call the office prior to coming to the office.
