Birth or 2 weeks– Hep B #1 if not given in the hospital
2 months – Pentacel #1 {DTap, IPV, Hib} Prevnar #1, Hep B #2, Rotateq #1
4 months – #2 Pentacel, Prevnar, Rotateq
6 months – #3 Pentcel, Prevnar, Rotateq
9 months – Hep B #3
12 months – Hep A #1, Prevnar #4
15 months – MMR #1, Varivax {Chickenpox} #1
18 months – Pentacel #4, Hep A #2 {must be 6 months from #1}
4 years – DTAP #5, IPV#4, Proquad #2{MMR & Varivax}
9 years and up – Gardasil can be given*Only 2 doses of Gardasil need to be given if first 2 doses are given prior to 15 years of age (doses need to be 6 months apart). Otherwise 3 doses are needed.
15 years and older need 3 doses:
1st dose – date given
2nd dose – 2 months later
3rd dose – 4 months after 2nd dose
12 years – Adacel {TDAP}, Menactra #1, Hep A if not given before
13-15 years – catch up on any shots not given
16 years – Menactra #2, and Bexsero #1, dose #2 can be given 1 month later
17-21 years – catch up on any shots needed
** Flu rules- 1st dose given as early as 6 months of age, then booster dose 1 month later for first time flu shot recipients. Only 1 flu shot needed yearly after that.
