Annual Well Child Checks and Sports Physicals
It's that time of year again when parents have to start thinking about their child's annual well check. For preteens and teens, this annual well check also counts towards a sports physical for the upcoming school year but only if done after May 1st so keep that in mind when scheduling your appointment. Please bring any forms that need to be completed for school, camp or sports with you to your child's appointment. Make sure you complete the health history questionnaire on the sports form as well prior to coming to our office. If your child has any medications at school, there is a form for that as well. Most forms can be found on your school's or your county's website.
