Upcoming change in practice

Dear Patients,
I hope this message finds you well. It is with mixed emotions that I write to inform you of an upcoming change in my practice. As of July 19, 2025, I will no longer be a provider with Privia Medical Group.

I want to express my deepest gratitude for allowing me to be a part of your healthcare journey during my time at Privia Medical Group.

I will be establishing a new practice at a future date. My new practice information will be:

Important information regarding this transition:

• After July 19, 2025, I recommend you continue your endocrinology care with another provider at Privia Medical Group or with a provider of your choice.
• The exact opening date of my new practice has not yet been determined and will depend on several factors including credentialing and insurance processes.
• For your ongoing endocrinology needs, please consult with your primary care physician or reach out to your insurance for a referral to another endocrinologist.
• Should you wish to see me at my new practice once it opens, you will need to verify your insurance coverage by calling your insurance provider.

If you need to request your medical records, you can call 346.999.2130 for assistance with the records transfer process. We will require written permission or authorization from you if you request your medical records.
