An important announcement about my practice

It is with mixed emotions and sincere gratitude that I am writing to inform you of a significant change in my professional life. After 25 wonderful years of private practice, I have decided to retire from independent practice, with my last day at my office being October 31, 2025.

I want to express my deepest thanks for the trust and loyalty you have shown me throughout the years. It has been an honor to be your OB/GYN, and I will always cherish the relationships we have built. 

I am pleased to share that I will not be retiring from medicine completely. Instead, I will be transitioning to a new role as an employee at HHM Women’s Health Center. This move will allow me to continue focusing on patient care within a supportive and collaborative environment. I will begin seeing patients there on December 1, 2025.

For a seamless transition, you have a couple of options regarding your ongoing care:

• To continue your care with me: I would be delighted to continue our relationship at HHM Health.  Your medical records are confidential, and with your permission, a copy will be available to me and the medical team at my new location. For your convenience, an authorization form is attached. Please sign and return it to my current office by October 31, 2025 to have your records transferred.
• To transfer your care to a new provider: We will gladly transfer copies of your medical records to a physician of your choice. Please complete the enclosed authorization form and return it to my office. We will forward your records promptly upon receiving the signed form. 

After October 31st, you can obtain your records by requesting them from MedicalRecords@priviahealth.com or faxing a request to 202-403-3099. Our office will be available to assist you any needs until October 31, 2025.
