Dearest patients,
It is with mixed emotions that I have decided to close my office permanently. My last day in the office will be August 29, 2025. I will have very limited visits in August while we work to close the office.
If you have an appointment already in August we will contact you to confirm your visit. If you are overdue on your wellness exam, we would be happy to schedule you from now until the end of July. Please call soon to get scheduled.
If you are pregnant and due before August 1st we will still see you for your post-partum visits in August. If you are due after August 1st and have any questions please call the office.
If I am currently monitoring any medications that you take on an ongoing basis please find another doctor who can manage this and fill out the record transfer form soon. I am providing a list of doctors whom I trust to continue to care for your medical needs.
We encourage you to establish your portal to have easy access to your records. Before 8/29/25 you may fill out the attached form and return to our office by email to info@behanobgyn.com or by fax to 214-987-1786. This is for records from 2020 to current. Any records prior to 2020 may incur a fee. A third party company will handle all record requests after 8/29/25 and more information will be provided on our website at www.behanobgyn.com.
