Who Should Enroll?

We started this program to help patients who are taking a proactive role in managing their diabetes, hypertension, lung disease, and other conditions. Not sure if you are eligible for the program? How Does It work?

Once you’re in the program, we will assign you a Care Coordinator, who will call you regularly to help manage your health and well-being. Your Care Coordinator will help you get medication refills and coordinate transportation, community resources, and medical equipment. This can all be done over the phone.
