Office visits are made on an appointment basis, we do not take walk-ins and we ask you call at least 24 hours prior to your appointment to cancel. As we do our best to honor your appointment time, we ask you to arrive a few minutes early to update any information we may need. New patients should arrive 15 minutes prior to their scheduled appointment time. Please bring your insurance card with you to each visit.
In case of an emergency, if you find it necessary to contact the doctors after regular office hours, you can call 203-734-1644 and leave a message with our answering service, all messages are forward to the doctor on call. If you don’t hear from our on-call provider within a reasonable time, do not hesitate to call again.
We ask that you request all prescription authorizations for renewals during office hours. When you are requesting a refill for a prescription, please have the name of the medication and dosage available, and allow at least 24 hours for request. For controlled substances we ask that you call at least 1 week prior to when your prescription is due to allow enough time for insurance authorizations if they are required.
Any form you need to filled out must have the parent or guardian’s part all filled out prior to drop off and we asked you to allow 3-5 days for return. If you wish them mailed, please attach a stamped, addressed envelope.
Medical records- we have a release form in our office or you can have one from the new provider sent to us. The process of medical records requires at least 3-4 weeks, they are loaded on a CD and there is a charge for them as well as postage if you would like them to be mailed. This process does not start until we have a signature from parent or guardian.
