**DR. BENJAMIN LEE NOW OFFERING COVID-19 VACCINES**

Dear Valued Patients,

One of the best ways to protect yourself from COVID-19, and also to help STOP the spread of COVID-19, is to get a vaccine as soon as possible!!

We are happy to announce that effective immediately, we will be offering COVID-19 vaccines at our office on certain evenings.

Please note, these vaccine clinics are NOT walk-in!  You need to be contacted by our office staff and have an appointment time.

**PLEASE ONLY COMPLETE THE SURVEY ONCE FOR EACH PATIENT.

**PLEASE DO NOT CALL OUR OFFICE WITH QUESTIONS ABOUT THE VACCINE CLINIC.

**WE WILL BE CALLING EVERYONE WHO REGISTERS IN ORDER THAT THEY SIGNED UP.

**IF YOU DO NOT GET CALLED THIS COMING WEEK, YOU WILL LIKELY BE CALLED IN THE NEXT 2-3 WEEKS.

**FINALLY, THIS VACCINE CLINIC IS ONLY FOR OUR PATIENTS. IF YOU HAVE A FRIEND OR FAMILY MEMBER WHO IS NOT OUR PATIENT, WE CANNOT GIVE THEM THE VACCINE.
