What should you know about PCMH?

The relationship between you, your provider, and the care team (MA’s and Nurses) is the core driving force behind Patient Centered Medical Home. At Kids Doc Pediatrics, we will continue to provide high-quality medical care that is right for your child based on evidence- based clinical guidelines and quality standards.

Your provider will continue to direct your care team based on your child's needs. To improve efficiency, the care team will plan for your appointment by reviewing the medical chart, checking for recent testing, and prompt notification of lab work and imaging in a timely manner. Your care team will work with any outside specialists/providers to coordinate healthcare across all specialties. We will continue to monitor your medical needs and contact you to schedule follow-up appointments at a time that is convenient for you.
