• A list of your prescriptions and your medical plan
• Contact information for your endocrinologist
• Your emergency care plan
• Your medical insurance information
• If possible, contact information for a medical provider in your area

• Does your health insurance cover you away from home?
• Do you need supplemental or travel insurance?
