Refills

Please call your pharmacy to have them send a refill request for all non-controlled medications.

Controlled medication refills require an appointment if there are no refills available. Please plan your appointments accordingly as a same day appointment may not be available for a medication refill. (examples: Adderall, Ambien, Clonazepam).

Please have all refill requests in by Friday at 11am to be filled before the weekend. They will not be filled over the weekend.

Hospitalizations

Please let us know you were hospitalized once discharged and fill out a records request form if you were admitted anywhere other than an Archbold hospital. It is helpful to have your records at your follow- up appointment. 

Sports Physicals

Dr. Millsaps offers FREE sports physicals twice a year at Cairo High School. She encourages you to attend these if needed. Announcements will be made. 

If your child is UP TO DATE on their yearly Well Child visit and it has been within the last 3 months, I will fill out the form for you without an appointment. If it has been more than 3 months, please attend one of the free sports physical sessions if possible. 

If your child is DUE for a yearly Well Child visit, then they will be scheduled for that and a sports physical will be completed at that time.
