Provider–Patient Agreement | Effective 1/2026

Welcome to our practice. We are committed to providing high-quality, efficient, and compassionate care. In order to maintain a smooth and respectful experience for all patients, we ask that you review the following expectations:

  • Appointment Timeliness
  • Please arrive on time for your scheduled appointment.

Late arrivals may require rescheduling if there is insufficient time to provide appropriate care.

We recommend arriving a few minutes early if paperwork or updates are needed.

Communication

We ask that patients utilize the patient portal and other approved communication platforms for appointment requests, prescription refills, and non-urgent questions.

Portal communication allows for secure, efficient responses and helps streamline your care.

Urgent medical concerns should not be submitted through the portal and should be directed to appropriate emergency services if needed.

Pre-Visit Registration

Patients are expected to complete pre-visit registration through Phreesia prior to their appointment.

Completing forms, insurance updates, and consent documentation in advance helps reduce wait times and improves the efficiency of your visit.

Financial Policy

We require participation in our card-on-file program or a deposit prior to services being rendered.

Copays, deductibles, coinsurance, and outstanding balances are due at the time of service.

Maintaining an updated payment method ensures a seamless billing process and allows us to focus on patient care. Unpaid balances are automatically moved to collections after 90 days.

Attendance & Cancellation Policy

We respect your time and reserve your appointment specifically for you.

We ask that you extend the same courtesy by providing appropriate notice if you need to cancel or reschedule.

Cancellations must be made within the required notice period (as outlined in our office policy).

Late cancellations or missed appointments will result in a fee.

Repeated missed appointments may affect the continuation of care within the practice.
