Biography

Rafael Fernández Bohórquez, MD, is a board-certified physician in Internal Medicine and Addiction Medicine with more than 15 years of experience providing comprehensive care to adults. He specializes in preventive medicine, chronic disease management, addiction treatment, and the integration of primary care with behavioral health and pain management.

Dr. Fernández has practiced in community health centers and large multispecialty medical groups across Massachusetts, Ohio, and Texas. He also served as Medical Director of an outpatient addiction medicine program, leading efforts to expand access to evidence-based treatment and coordinated patient care.

Fluent in English and Spanish, Dr. Fernández is committed to delivering compassionate care in a welcoming and inclusive environment. He partners with patients from all backgrounds—including LGBTQ+ individuals—to support their unique health needs and long-term wellness goals. His approach emphasizes collaborative decision-making to help patients achieve lasting improvements in health and quality of life.
