Before submitting your request, please remember:

• Complete the Patient Request Form and upload it through the Patient Portal along with blank copies of all required school forms.
• If submitting forms for multiple family members, be sure you are logged into the correct patient's portal account before uploading.
• A form completion fee applies. Please ensure you have an active credit card on file for payment.
• Patients must be seen routinely by their provider for form requests to be processed. If the patient has not been seen recently, an appointment may be required before forms can be completed.
• Forms take 5-7 business days to complete, thank you for your patience.
