Regardless of the location your surgery is at, you will need to complete the Pre-Op Clearance Form. Please download and complete the form that matches your age group.


If you are 50 years or older, you will also need to get an EKG and CBC less than 30 days prior to your procedure.

  1. Fill out the medical history form online at www.onemedicalpassport.com as soon as possible.
  2. After completing the form, call either (571) 209-6441 or (571) 209-6440 to speak with one of the Pre-op Interview nurses.
  • During the interview, your medical history will be reviewed, and instructions will be provided to prepare for your procedure.

Note: Check with your surgeon/provider to see If you may need lab work or an EKG before surgery. If so, It Is Important that you fax any test results or other test information to (571) 206-3938 before the day of surgery.

The type of anesthesia you have determines what you need to do to prepare for surgery. The preparation below Is for General, Regional, Epidural, Spinal, or Local with Monitored Anesthesia Care (Local MAC).

DO NOT:

  • Eat/drink after midnight the night before surgery
  • Apply lotion to your skin after showering before your surgery
  • Wear makeup/jewelry to your surgery
  • Wear contact lenses (It Is recommended you wear your glasses)

YOU MAY: 

  • Bathe/Shower before arriving to reduce bacteria on your skin
  • Brush your teeth
  • Take prescription(s) with a sip of water

Please ensure you bring only your picture ID, insurance card(s), and any co-pays that are applicable. You will not be able to drive for 24 hours post-surgery, so ensure that you have an adult (18 or older) accompanying you who can drive you home safely. Transportation by taxi is acceptable if another responsible adult still accompanies you.

  1. Arrive at least one hour before your scheduled surgery. Upon entering the building, take the elevator to the 2nd floor, Suite 200.
  2. Proceed to the sign-in desk to check in— have your photo ID and insurance card ready, as well as any applicable co-pays.
  3. At the time of surgery, you will be escorted into the preoperative area, where you will change into a gown, cap, and slippers.
  4. To keep you comfortable, a small dose of local anesthetic will be applied before starting your IV.
  5. After preparation, you will be escorted to the operating room by your anesthesia provider and RN.
  6. Ask any questions as they come up!

  1. You will be taken to the Post Anesthesia Recovery Unit, where you'll be monitored until the anesthesia has worn off.
  2. You may begin to experience some discomfort and will be asked to rate your level of pain on a scale of 0 (no pain) to 10 (severe pain). Pain-control medication will be given accordingly.
  3. You will then be moved to the second-stage recovery unit, where a family member/friend can join you.
  4. When you feel well enough to be discharged, a nurse will review instructions specific to your surgery & anesthetic, and you will be given a written copy.
  5. A staff member will escort you to your car, and a trusted adult must accompany you and drive you home.
  6. You will receive a follow-up call from a surgery center staff member, 1-2 business days after your procedure, to ensure there are no problems and answer any questions.

Your surgeon will: 

Schedule the date & time of your procedure with the Surgical Services Scheduler and determine whether you will be an inpatient or outpatient.

  • Inpatient: You will be admitted to the hospital and remain in the hospital for 1 or more days until you're ready to go home.
  • Outpatient: You will come to the hospital the day of your operation and go home that same day.

You’ll receive instructions on food/medication rest on food/medication restrictions before your surgery. In the event your procedure needs to be rescheduled/canceled, contact the surgical services scheduler.

You will: 

  1. Receive a call from the Pre Surgical Services staff to schedule a phone interview with a nurse who'll review your medical, surgical, & family history. They will also discuss any additional testing needed and dietary restrictions required by anesthesia and answer any questions you may have.
  2. Prepare for the phone interview by listing information on pertinent medical history (specialists you have seen, previous procedures you have had with anesthesia, allergies, and medications including the dosage and times taken. You should expect the interview to last 30 minutes to an hour.
  3. Plan for a responsible adult to accompany you home post-surgery. Public transportation (i.e. Bus or taxi) is not a satisfactory method of transportation.
  4. Receive a call from the registration staff who will verify your insurance, demographic information and co-pay if applicable. Please have your insurance card information ready.
  5. If you are scheduled for hip or knee replacement surgery, call (703) 858-6768 or (703) 858-8025 to reserve a space at LHC’s joint camp.
  6. Follow and complete your procedural preparation if ordered.
  7. Receive a call the day before your surgery from the surgical services staff confirming your expected arrival time on the day of your surgery.

Please Note:

  • If you become ill or have a fever, please call your surgeon.
  • If you need an interpreter, one will be scheduled to join for the phone interview.

DO NOT:

  • Eat or drink anything after midnight the night before surgery.
  • Apply lotion to your skin after bathing
  • Wear makeup or jewelry
  • Wear contact lenses (it’s recommended you remove them and wear your glasses)
  • Bring any valuables

YOU MAY:

  • Take your prescription medication as directed by the interview nurse
  • Brush your teeth, but do not swallow any water
  • Bath or shower before arriving with an antibacterial soap (recommended you do this the night before/morning of surgery to reduce bacteria on skin)

Please ensure you have these ready to bring to your surgery:

  • Photo ID
  • Insurance card
  • A list of medications with dosages and how often you take the medication
  • Amount quoted to meet your deductible, co-pays, and/or co-insurance
  • Telephone number of adult driver or escort taking you home

You will not be able to drive for 24 hours, so an adult 18 years or older must accompany you and drive you home to ensure your safety. Transportation by taxi is acceptable if another responsible adult accompanies you. Please plan on having your driver remain with you for the entire procedure if possible. A parent or legal guardian must always remain in the facility when children under the age of 18 are undergoing procedures at the facility.

  1. Arrive at the surgery center at least one hour before your scheduled surgery. Free parking is available in front of the building.
  2. Upon entering the building, proceed by elevator to the second floor, suite 200. Proceed to the registration desk to check in. Please bring your photo ID and your insurance card with your co-payment if applicable. If you have insurance questions, call (571) 209-6443.
  3. Proceed to the Surgical Services waiting area. The Pre-Operative Team will be notified you are there, and your Preop room will be prepared.
  4. Once your room is ready, a team member will come out to escort you to your room. A member of your family/companion may accompany you if you wish. a. Your family/companion will be asked for their contact number and directed to your assigned room while your weight is obtained.
  5. When you reach your room, you will be asked to change into a hospital gown.
  6. You will meet the Surgical Team who will verify your name, procedure, review your medical information, and obtain your Surgical Consent.
  7. When the procedure is ready to begin, you will be wheeled on a stretcher to the Operating Room by the Operating Room Nurse and Anesthesiologist or Certified Registered Nurse Anesthetist (CRNA).
  8. Your companion or family will be directed to proceed to the Surgical Services Waiting Area. An overhead flat screen monitor is provided for families and companions to track your procedure progress by viewing the status board in the waiting area.

When your procedure is completed,

  1. You will be taken to Phase I Recovery.
  2. If you are admitted to the hospital after surgery, the recovery room nurse will call your family/companions when you are ready for transfer to the inpatient nursing unit. Family/companions may then proceed to your room and will see you once you are in the nursing unit.
  3. If you are going home after your procedure, you will be moved to the Phase II recovery area when awake from surgery.
    • The family member or companion you listed as contact will be called to come back to your Phase II recovery room when you are settled, comfortable and ready.
    • While in Phase II, you and your family/companion will be provided discharge instructions and any prescriptions the surgeon has written.
    • You will be ready to go home when certain discharge criteria are met (no nausea/vomiting, stable vital signs, tolerating oral liquids, pain under control).

You will be contacted by a nurse prior to your surgery to review your health history, medications, pre- operative instructions, and any coinsurance due for your surgery.

Note: Notify your surgeons if there is a change in your physical condition (cold, fever, respiratory problems, etc.).

DO NOT: 

  • Eat/drink anything after midnight the night before your operation, including hard candy or cigarettes
  • Take any medications after midnight unless instructed by your surgeon/nurse
  • Wear makeup/jewelry (including body piercings), or body spray to your surgery
  • Wear contact lenses your surgery (please wear your glasses instead)
  • Bring rings, watches, or other valuables to your surgery

DO: 

  • Tell your surgeon if you are on any type of blood thinners/aspirin
  • Arrange for a responsible adult to accompany you to the PoR Surgery Center and remain with you the first 24 hours after surgery

Please ensure you have these ready to bring to your surgery: 

  • Photo identification and your insurance card(s). Our staff will need to verify and make copies
  • when you check in on the day of your surgery.
  • Any medications that you may need during your stay at the facility (e.g., inhaler or insulin).
  • A list of all drugs you are currently taking.
  • Payment of any patient responsibility (e.g. co-payment or deductible).
  • Please do not bring rings, watches, or other valuables.

  • Wear loose, comfortable clothing that is large enough to accommodate a bandage after surgery.
  • Wear comfortable shoes, such as slip-ons.
  • You will need to change into a surgical gown once in the pre-op area.
  • If your child is having the surgery, feel free to bring a favorite stuffed animal/security blanket for added assurance.

  • You will be discharged to your car by wheelchair.
  • If anesthesia was administered, you must have a responsible adult present to drive you home and care for you following surgery.
  • Your physician will provide post-operative instructions regarding diet, rest, exercise, and medications.
  • You will be provided with a written summary of these discharge instructions.
  • You will receive a call from a nurse the day after your surgery to check on your progress and discuss any questions you may have.
  • Point of Rocks Surgery Center will submit your bill to your insurance company, and you will receive a separate bill from your doctor, anesthesiologist and/or pathologist.

  • You will be contacted by a nurse prior to your surgery to finalize arrangements, complete a health history, and answer any questions you may have.
  • A representative will help you complete all needed insurance information. Prince William Surgery Center accepts most major insurance plans, including Medicare.
    • If you have questions regarding your coverage, you may call the billing specialist (571) 229-1525
  • Remember to schedule your pre-testing. Ask your doctor prior to your surgery for your post operative medication prescription. If you notice a change in your health (cough, cold, fever, etc.), it is important you notify your physician before your scheduled surgery, as well as the Prince William Surgery Center at (571) 229-1536, extension 224.

Please ensure you limit the intake of food and fluids before your surgery.

DO NOT:

  • Smoke/drink alcoholic beverages 24 hours before your surgery
  • Eat/drink anything after midnight the night before your surgery, unless directed by your physician
  • Bring rings, watches, or other valuables
  • Wear contact lenses to your surgery (please wear glasses and bring a case)

Please ensure you have these ready to bring to your surgery:

  • A photo identification and your insurance card(s). Our staff will need to verify and make copies when you check in on the day of your surgery.
  • Any medications that you may need during your stay at the facility (e.g., inhaler or insulin).
  • A list of all prescription medications you are currently taking and their dosages.
  • Payment of any patient responsibility (e.g. co-payment or deductible).
  • Your COVID vaccination card if you are vaccinated.

  1. You will have received instructions regarding arrival time from your pre-operative phone call. It is important you arrive at that designated time.
  2. You must be accompanied by a responsible adult who will drive you home following your surgery.
  3. When you arrive at the facility, you will be checked in by staff. The admission process is usually very quick as they will have obtained most of your information prior to your arrival. This final check allows us to verify all your key information so we can better serve you.
  4. Post check in, a nurse will conduct a pre-operative assessment that will include taking your vital signs and starting an IV if it is required for your procedure. The anesthesia provider will also speak with you in the pre-operative area to review all pre-operative information and discuss your anesthesia. You will have the opportunity to ask any questions regarding this.
  5. Our staff will keep your family and friends informed of your progress. We understand the anxiety family and friends will have while you are having your procedure. We will make every effort to keep them informed of your progress and when they will be able to re-join you after the procedure.

  1. You will be taken to a recovery room. A nurse will monitor your vital signs and make sure you are comfortable as the anesthesia begins to wear off. Once you are awake and alert, your family will be invited back to the recovery area. Special arrangements are made for children having surgery.
  2. The length of time you will need to recover depends on the nature of your surgery. Most patients are ready for discharge after one or two hours of rest.
  3. When you’re ready to go home, you’ll receive instructions regarding your post-operative care. These instructions will include information about medications, diet, and rest requirements.
  4. For your safety, it is crucial that you do not drive, operate machinery, or drink alcoholic beverages for at least 24 hours following your surgery. You must have the necessary transportation arrangements for after your procedure with a responsible adult.
