ATTENTION PATIENTS

This letter is to notify you that Sarah Hallbauer, M.D., will no longer be practicing with Family Medicine Associates of Conroe, effective Monday, November 16, 2026. You may continue receiving care in our office, or you may request that a copy of your medical records be transferred to another healthcare provider of your choice.

Your medical record will remain with Family Medicine Associates of Conroe. However, to request a copy of your records or a transfer of your records, please contact HealthMark Group at 1-800-659-4035 between 8 AM – 5 PM, Monday – Friday.

Dr. Sarah Hallbauer will be joining Lone Star Family Health Center, 605 S. Conroe Medical Drive, Conroe, TX 77304. We wish her the very best in her new pursuits!
