At Julie Tomberlin MD PA, we strive to make insurance and billing as clear and straightforward as possible for our families.

Insurance Plans We Accept 

We accept many major commercial insurance plans, including: 

  • Blue CrossBlue Shield (all states)
  • United Healthcare
  • Aetna
  • Cigna
  • Baylor Scott & White
  • Galaxy
  • HealthSmart
  • Humana
  • MultiPlan
  • Sana 

HMO Plans:

We only accept Blue Cross Blue Shield HMO plans. 

If you have an HMO plan through another insurance company, we are not in network. 

Important: 

Insurance plans vary, and we are not in network with every plan under these companies. We strongly recommend contacting your insurance company so they can help you determine if we are in network with your specific plan.


Payment Policy

  • Payment is required prior to being seen for your visit
  • This includes copays, deductibles, or self-pay balances

Deductible Plans: 

For patients with deductible plans, we require a $125 upfront payment at the time of the visit. After your insurance processes the claim, additional balance may be owed depending on your specific plan benefits. 

We accept: 

  • Credit cards (Visa, Mastercard, American Express, Discover)
  • HSA/FSA cards
  • Apple Pay
  • Google Pay

Card-on-File Policy

To simplify billing and reduce paperwork, we require a card on file

At check-in: 

Your preferred card (including HSA/FSA) will be securely stored 

After your insurance processes your claim: 

  • You will receive notification of any balance owed
  • The balance will be charged to your card on file
  • A receipt will be emailed to you 

Your card information is securely stored through our payment processor and is not kept on our office systems, in accordance with PCI-DSS security standards.


Fees

Missed Appointments & Cancellations 

  • Missed sick visit: $50
  • Missed well visit or behavioral visit: $100
  • Late cancellation (<24 hours): $25 

Patients may be dismissed after repeated missed appointments or late cancellations.


Self-Pay Patients 

For families who are not using insurance: 

  • Payment is required prior to the visit
  • We offer a self-pay discount
