Dr. Brent Fowler Practice Closing Notice

Dear Patient,

I am writing to inform you that I will be closing my practice at Stephenville Medical & Surgical Clinic (SMSC), the Clinic at Glen Rose (CGR), and Community Health Clinic (CHC) effective October 30, 2026. I have accepted a university athletic medical directorship with the University of Wyoming, where I will serve as the lead team physician for Wyoming Athletics and partner with their sports medicine fellowship.

To ensure a smooth transition, I encourage you to begin the process of selecting a new primary care physician. If you need assistance with this, we are happy to provide referrals or guidance based on your needs.

Your medical records will remain here at SMSC/CGR/CHC. If you would like your medical records transferred to a provider outside of our clinics, please contact HealthMark Group customer service online at https://requestmanager.healthmark-group.com.
